Patient Bill of Rights and Responsibilities

At Infusion for Health, our mission is to transform and elevate patient care by providing services that are patient-focused, cost-effective, and compassionate. To accomplish this goal, we must partner with you, the patient, and work as a team, each with rights and responsibilities. 

As a patient, you have the right to: 

  1. Be fully informed, in advance, about care or services provided and any limitations to those services. 
  2. Be informed, in advance of service, of your financial responsibility. 
  3. Participate in the development and periodic revision of the plan of care or service. 
  4. Refuse care or treatment. 
  5. Formulate an Advanced Directive and receive information concerning Advance Directives if requested. 
  6. Be treated with respect, consideration, dignity, and individuality, and to receive appropriate care without discrimination. 
  7. Identify visiting personnel. 
  8. Receive care or service that is free from mistreatment, neglect, or verbal, mental, sexual, and physical abuse, and misappropriation of patient property. 
  9. Voice grievances or complaints without fear of reprisal and have them thoroughly investigated. 
  10. Confidentiality and privacy of all your patient information. 
  11. Choose a provider, if applicable. 
  12. Be informed of the provider’s responsibilities. 

As a patient, you have the responsibility to: 

  1. Submit the forms that are necessary to receive services. 
  2. Provide accurate medical and contact information and report any changes as they occur. 
  3. Notify the treating provider of your consent to participate in the care or services provided by the facility. 
  4. Notify the facility of any concerns about the care or services provided.
  5. Help maintain a safe and supportive environment by interacting respectively with everyone in our clinic.
